Follow these easy steps in order to a patient of the Vaughan Medical Centre Natural Healthcare Team.

STEP 1: Please fill out all FOUR (4) online intake forms below. The "secured appointment form" is important in order to guarantee you an appointment. No charges will be made to your credit card at this time. Charges will only apply if an appointment is missed without advance notice. Without this form filled out, there is no guarantee that your appointment will be made or held. Please allow us 3 business days to respond to you with scheduling options. Do not forget to bring all current laboratory, diagnostics, and other information on your condition(s) with you to your first appointment.

STEP 2: Please PRINT OUT and SIGN the "Informed Consent" and the "Consent for Disclosure of Information" and bring them with you to your appointment.
